Job Description:

Aveanna Healthcare is seeking a compassionate and skilled Private Duty LVN to provide one-on-one care to our medically fragile patients in the Three Rivers and George West, TX area.

About the Role:

• 
  
• Provide continuous care to patients in their homes, assisting with transitioning from hospital to home and promoting independence.
  
• Implement physicians' prescribed plans of care, coordinate patient assessments, and administer prescribed medication, treatments, and therapies.
  
• Compliantly document care provided in our electronic system of record and promote health education to patients and their families.
  
• Collaborate with our multidisciplinary team to ensure seamless patient care and satisfaction.
  
  
  Responsibilities:
  
  
  • 
    
  • Assess patient needs, develop and implement personalized care plans, and provide ongoing education and support to patients and their families.
    
  • Administer medications, treatments, and therapies as prescribed by physicians and document patient care in our electronic record system.
    
  • Coordinate with healthcare providers to ensure timely and effective patient care, and participate in ongoing quality improvement initiatives.
    
  • Participate in continuing education and professional development to enhance patient care and stay up-to-date on industry best practices.
    
    
    Qualifications:
    
    
    • 
      
    • Active, unencumbered LVN license in the state of Texas.
      
    • Current CPR certification from a reputable organization, such as the American Red Cross or American Heart Association.
      
    • 6 months prior hands-on nursing experience, with a strong desire to make a positive impact on the lives of our patients.
      
    • Reliable transportation and a valid driver's license.
